Release of Liability Form is Required for Every Rider
All riders must have a signed "Release of Liability" form on file with the Quail Roost Riding School Instructor. This is our adaptation of the standard form developed by the State of North Carolina. This form also provides us with important information, including your address, phone numbers, and e-mail address. If more than one family member joins the lesson program, please do not put on same form, each customer must have it's own release form. Please keep us updated on any address or phone number changes so we can get a hold of you if necessary.
